Will my insurance cover Fosfomycin Tromethamine?

Fosfomycin tromethamine is not routinely covered by most health insurance plans. When coverage is available, the medication is often assigned to a higher copay tier, resulting in significant out-of-pocket expenses. If your plan does cover it, check your policy documents or contact your insurer to learn your specific copay amount. Patients may also explore pharmacy discount programs as an alternative way to reduce costs.

Is Fosfomycin Tromethamine covered under Medicare Part D?

Some Medicare Part D plans do include fosfomycin tromethamine on their formulary, but many do not. When the medication is covered, it typically falls into a high copay tier. Combined with deductibles and other out-of-pocket costs, Medicare coverage may still result in substantial expense. Comparing pharmacy prices or using discount programs can offer a more affordable option for Medicare beneficiaries.

What is the brand name for this medication?

Fosfomycin tromethamine was originally marketed under the brand name Monurol, but no branded version is currently available in the U.S. market. The medication is sold only as a generic and is produced by a limited number of manufacturers, which may contribute to its higher price compared to more widely available antibiotics.

Fosfomycin tromethamine is a generic prescription antibiotic approved by the FDA for treating acute cystitis (bladder infections) in women. Patients can significantly reduce this expense by comparing prices across different pharmacies or using prescription discount programs.

Warnings & interactions

This is a summary of the official FDA label text, not personalized medical advice. It does not list every possible interaction or side effect. Always tell your prescriber and pharmacist about every medicine and supplement you take.

Contraindications

CONTRAINDICATIONS Fosfomycin tromethamine is contraindicated in patients with known hypersensitivity to the drug.

Warnings

WARNINGS Clostridium difficile associated diarrhea (CDAD) has been reported with use of nearly all antibacterial agents, including fosfomycin tromethamine, and may range in severity from mild diarrhea to fatal colitis. Treatment with antibacterial agents alters the normal flora of the colon leading to overgrowth of C. difficile . C. difficile produces toxins A and B which contribute to the development of CDAD. Hypertoxin producing strains of C. difficile cause increased morbidity and mortality, as these infections can be refractory to antimicrobial therapy and may require colectomy. CDAD must be considered in all patients who present with diarrhea following antibiotic use.

Known drug interactions

DRUG INTERACTIONS Metoclopramide: When coadministered with fosfomycin tromethamine, metoclopramide, a drug which increases gastrointestinal motility, lowers the serum concentration and urinary excretion of fosfomycin. Other drugs that increase gastrointestinal motility may produce similar effects. Cimetidine: Cimetidine does not affect the pharmacokinetics of fosfomycin when coadministered with fosfomycin tromethamine.

Pregnancy & breastfeeding

PREGNANCY Teratogenic Effects When administered intramuscularly as the sodium salt at a dose of 1 gram to pregnant women, fosfomycin crosses the placental barrier. Fosfomycin tromethamine crosses the placental barrier of rats; it does not produce teratogenic effects in pregnant rats at dosages as high as 1000 mg/kg/day (approximately 9 and 1.4 times the human dose based on body weight and mg/m 2 , respectively).

Does Medicaid pay for Fosfomycin Tromethamine?

Coverage under Medicaid is inconsistent and varies by state. While some state Medicaid programs do include fosfomycin tromethamine, many do not. Individuals enrolled in Medicaid who cannot obtain approval for this medication should explore prescription discount programs or speak with their healthcare provider about alternative antibiotic options that may be covered.

What are my options for reducing the cost of Fosfomycin Tromethamine?

Because fosfomycin tromethamine is a rarely prescribed and higher-cost antibiotic, several cost-reduction strategies exist. Comparing prices across different pharmacies can reveal significant variation in the per-packet cost. Patients may also qualify for Medicaid or Medicare Extra Help programs, or seek assistance through a local 340B clinic or hospital. Nonprofit patient assistance programs sometimes offer support as well. As a generic medication, manufacturer coupons are typically unavailable. Speaking with your healthcare provider about why this specific antibiotic was chosen may also reveal whether a less expensive alternative would be equally effective for your condition.

Are there less expensive antibiotics available to treat the same infection?

Fosfomycin tromethamine is not a first-line UTI treatment, and several other antibiotics are generally more affordable. Common, less expensive alternatives include generic sulfamethoxazole-trimethoprim, cephalexin, nitrofurantoin, and ciprofloxacin. Before switching medications, consult your healthcare provider, as individual factors—such as allergies, drug interactions, or antibiotic resistance patterns—may influence which option is most appropriate for your situation.
