What is Lincomycin used for?
Lincomycin is the original lincosamide antibiotic — clindamycin, its far more widely used chemical relative, was developed later specifically to improve on lincomycin's absorption and side-effect profile, which is why clindamycin has largely replaced lincomycin in modern practice. Lincomycin is still used by injection for serious streptococcal, pneumococcal, and staphylococcal infections, but specifically reserved for patients who are allergic to penicillin or for whom a penicillin is otherwise unsuitable, rather than used as a first-line choice.
Lincomycin carries the same boxed warning that made clindamycin famous for caution: a significant risk of Clostridioides difficile-associated diarrhea (CDAD), ranging from mild diarrhea to fatal colitis, because disrupting the colon's normal bacterial flora lets C. difficile overgrow. This risk is a defining feature of the entire lincosamide class, and it's the reason lincomycin's label explicitly states it should be reserved for serious infections where less toxic alternatives aren't appropriate — not used for minor infections or upper respiratory illnesses, most of which don't need this risk profile at all.
A less commonly discussed but clinically real property is that lincomycin has neuromuscular blocking activity of its own, which can enhance the effect of other neuromuscular blocking agents used during anesthesia — anesthesiologists and surgical teams need to know if a patient has received lincomycin, since it can prolong or intensify paralysis from these drugs. It's contraindicated in patients with known hypersensitivity to either lincomycin or clindamycin, reflecting the close chemical relationship between the two.

Blocks bacterial protein synthesis
Lincomycin binds to the bacterial ribosome and interferes with the machinery bacteria use to build proteins.
Stops bacterial growth
Without functioning protein synthesis, susceptible bacteria — including streptococci, pneumococci, and staphylococci — can't grow or multiply.
The infection clears
With bacterial growth halted, the immune system clears the remaining organisms and the infection resolves.

Because lincomycin therapy has been associated with severe colitis which may end fatally, it should be reserved for serious infections where less toxic antimicrobial agents are inappropriate, as described in the INDICATIONS AND USAGE section. It should not be used in patients with nonbacterial infections such as most upper respiratory tract infections. C.

Clostridioides difficile associated diarrhea Clostridioides difficile associated diarrhea (CDAD) has been reported with use of nearly all antibacterial agents, including Lincomycin, and may range in severity from mild diarrhea to fatal colitis. Treatment with antibacterial agents alters the normal flora of the colon leading to overgrowth of C. difficile . C. difficile produces toxins A and B which contribute to the development of CDAD. Hypertoxin producing strains of C. difficile cause increased morbidity and mortality, as these infections can be refractory to antimicrobial therapy and may require colectomy.
