Pivya (pivmecillinam) is a genuinely new addition to the US antibiotic landscape — it's the first amidinopenicillin approved in the US, though the parent drug mecillinam has been used in Europe for decades. It's approved specifically for uncomplicated urinary tract infections in women 18 and older caused by E. coli, Proteus mirabilis, and Staphylococcus saprophyticus, and its arrival matters because it gives prescribers another first-line oral option at a time when resistance to older UTI standbys (trimethoprim-sulfamethoxazole, fluoroquinolones) has been rising in many parts of the US. Mechanistically it's distinct from other penicillins — it binds a different penicillin-binding protein (PBP2 rather than PBP1/3), which is part of why it retains activity against some bacteria that have become resistant to amoxicillin or other beta-lactams.

The most important safety issue is specific to how the drug is metabolized: pivmecillinam is a "pivalate-generating" prodrug, meaning its breakdown releases pivalate, which depletes the body's carnitine stores. This is why it's contraindicated in patients with primary or secondary carnitine deficiency (including certain inherited metabolic disorders) and why it shouldn't be combined with valproic acid or other pivalate-generating drugs — the combination can accelerate carnitine depletion enough to cause hypoglycemia, muscle aches, fatigue, or confusion. It's also contraindicated in acute porphyria and in patients with a history of serious hypersensitivity to penicillins or cephalosporins, including Stevens-Johnson syndrome.

Because Pivya was only recently approved, it remains brand-only with no generic yet available, and its price reflects that — it's considerably more expensive than older first-line UTI antibiotics like nitrofurantoin or trimethoprim-sulfamethoxazole, so it tends to be reserved for cases where those options aren't appropriate due to allergy, resistance, or intolerance rather than used as a routine first choice.

4. CONTRAINDICATIONS Serious hypersensitivity reactions (e.g., anaphylaxis or Stevens-Johnson syndrome) to PIVYA or to other beta-lactam antibacterial drugs (e.g., penicillins and cephalosporins). ( 4.1 ) Primary or secondary carnitine deficiency resulting from inherited disorders of mitochondrial fatty acid oxidation and carnitine metabolism, and other inborn errors of metabolism (e.g., methylmalonic aciduria, or propionic acidemia). ( 4.2 ) Acute porphyria.

Pregnancy & breastfeeding

8.1 Pregnancy Risk Summary Published observational studies on PIVYA use during the first trimester do not indicate an increased risk of major birth defects ( see Data ). There are limited studies on PIVYA use during pregnancy that evaluate the risk of miscarriage and other adverse maternal or fetal outcomes. These studies have methodological limitations hindering interpretation. No dose adjustment is required in pregnant women (see Clinical Considerations) .
