Bicillin C-R 900/300 is a lower-strength version of Bicillin C-R, combining the same two components — penicillin G procaine and penicillin G benzathine — but in a 900,000/300,000-unit ratio rather than the standard product's higher combined dose. That distinction matters clinically: the 900/300 strength is formulated primarily for pediatric dosing, where a smaller total dose and injection volume is appropriate, while the standard-strength product is used more often in adolescents and adults.

Otherwise it works the same way as its sister product: the procaine component provides faster initial antibacterial activity, and the benzathine component releases slowly to sustain drug levels over an extended period — which is why a single injection can substitute for several days of oral antibiotic therapy for suitable infections, such as streptococcal pharyngitis.

The same boxed warning applies here as with all penicillin G benzathine products: it must never be given intravenously. Reports of inadvertent IV administration have been linked to cardiorespiratory arrest and death, so this injection is given only intramuscularly, by a trained healthcare professional.

CONTRAINDICATIONS A previous hypersensitivity reaction to any penicillin or to procaine is a contraindication.

Pregnancy Teratogenic effects Reproduction studies performed in the mouse, rat, and rabbit have revealed no evidence of impaired fertility or harm to the fetus due to penicillin G. Human experience with the penicillins during pregnancy has not shown any positive evidence of adverse effects on the fetus. There are, however, no adequate and well-controlled studies in pregnant women showing conclusively that harmful effects of these drugs on the fetus can be excluded.
