What is Cefepime used for?
Cefepime is a fourth-generation cephalosporin, meaning it covers a broader range of bacteria than earlier cephalosporin generations, including many gram-negative organisms like Pseudomonas aeruginosa that first- and second-generation cephalosporins don't reliably reach. It's given only by injection or IV infusion, and the FDA has approved it for pneumonia, urinary tract infections, uncomplicated skin infections, complicated intra-abdominal infections (paired with metronidazole), and as empiric therapy for fever in patients with neutropenia — a low white blood cell count that leaves them especially vulnerable to serious infection, often during cancer treatment.
An important safety issue clinicians watch closely with cefepime: it's been linked to neurotoxicity, including confusion, altered mental status, and seizures, particularly when the dose isn't properly reduced for a patient's kidney function. This risk is a key reason kidney function is checked before and monitored during cefepime therapy, with the dose adjusted accordingly.
