Clindamycin phosphate is the topical form of the antibiotic clindamycin, formulated as a gel or solution specifically for acne — sold under the brand name Clindagel, among others, and available in both once-daily and twice-daily generic versions. Applied directly to the skin, it reduces the population of Cutibacterium acnes bacteria and calms the inflammation that drives inflammatory acne lesions.
Even though it's applied topically, enough clindamycin can be absorbed through the skin to carry some of the same risks as the oral and injectable forms — most notably, the FDA label warns that topical clindamycin has been associated with severe colitis caused by C. diff, even though this is rare with topical use. Because of that, it's not recommended for people with a history of ulcerative colitis, Crohn's disease, or a previous antibiotic-associated colitis.
