Bacterial STIs are curable with the right antibiotic — but the CDC-recommended drug and dose is specific to each infection, so testing and diagnosis come first.

Several of the most common STIs — chlamydia, gonorrhea, syphilis, and trichomoniasis among them — are bacterial (or, for trichomoniasis, a parasite treated with similar drugs) and curable with antibiotics, unlike viral STIs such as herpes or HIV, which are managed but not cured.

Doxycycline is now the CDC-preferred treatment for chlamydia in most adults (a 7-day course), having largely replaced single-dose azithromycin due to better effectiveness in some infection sites. Azithromycin is still used in specific situations, including in pregnancy. Gonorrhea treatment has shifted over the years as resistance patterns changed — current CDC guidance recommends a single injectable dose of ceftriaxone, often no longer paired with azithromycin the way it once was. Gonorrhea develops from infection by the bacterium Neisseria gonorrhoeae. As a sexually transmitted infection, it spreads through vaginal, oral, or anal contact and can affect the genitals, anus, and mouth. Transmission from mother to baby during delivery is also possible. Young adults experience the highest rates of infection. Though some people have no symptoms, others develop painful urination, abnormal genital discharge, and intermenstrual bleeding. If left untreated, gonorrhea in women can progress to pelvic inflammatory disease and cause infertility.

Because gonorrhea is caused by bacteria, antibiotic therapy is necessary. If you've received a gonorrhea diagnosis, you may wonder whether amoxicillin—a widely recognized antibiotic—can treat it. The answer is no. Gonorrhea bacteria have acquired resistance to amoxicillin, rendering the drug ineffective. Current medical practice instead calls for a single intramuscular injection of ceftriaxone, a third-generation cephalosporin antibiotic.

Can amoxicillin cure gonorrhea?

Amoxicillin is a popular antibiotic used for numerous bacterial conditions, including respiratory and urinary tract infections. However, it has no role in gonorrhea treatment. According to Soma Mandal, MD, an internist at Summit Health, "Amoxicillin is not recommended as an effective treatment for gonorrhea." The reason: while amoxicillin and other antibiotics once worked against gonorrhea, they no longer do. "Gonorrhea, caused by the bacterium Neisseria gonorrhoeae, has developed resistance to many antibiotics that were previously effective, including amoxicillin," Dr. Mandal explains.

Resistance extends beyond amoxicillin alone. The CDC notes that gonorrhea has also become resistant to fluoroquinolones and azithromycin. Currently, ceftriaxone is the only antibiotic reliably effective against gonorrhea.

As Eboni January, MD, OB-GYN and fellow of the American College of Obstetricians and Gynecologists, explains, the gold standard is a single intramuscular injection of ceftriaxone. "The CDC and WHO (World Health Organization) recommend ceftriaxone as the first-line treatment because of its effectiveness and the resistance pattern of gonorrhea," says Dr. January. "Ceftriaxone covers most strains of gonorrhea, and its intramuscular administration ensures high tissue concentration, which is particularly important for clearing the infection."

"In cases where chlamydia coinfection is suspected, doxycycline (100 mg twice daily for 7 days) is also commonly prescribed, as these two sexually transmitted infections often occur together," she explains. But there isn’t a scenario where you would be prescribed amoxicillin for gonorrhea, Dr. January emphasizes.

Recommended amoxicillin dosages

Because amoxicillin is essentially never prescribed for gonorrhea today, no current dosage guidance exists for this use.

Historically, had amoxicillin been used—though not recommended given modern resistance patterns—treatment would have involved high doses. "If amoxicillin were used (although this is not recommended due to resistance), older regimens would have required high doses (e.g., 3 grams) taken with probenecid to slow renal clearance," explains Dr. January. "However, this is far from current practice."

Instead, the CDC specifies that ceftriaxone for uncomplicated gonorrhea of the genitals, anus, or throat is 500 mg delivered as a single intramuscular injection. This remains the only CDC-endorsed treatment for these cases.

How quickly does treatment work?

Amoxicillin is not used to treat gonorrhea, but if it were used, it would take several days to work, Dr. January says. But, "given the resistance patterns of Neisseria gonorrhoeae, it may not be effective at all," she says.

With ceftriaxone, the recommended therapy, improvement typically begins within days. "You'll know the antibiotic is working when symptoms, such as pain during urination, genital discharge, or pelvic discomfort, begin to improve," Dr. January describes.

Some healthcare providers order a "test of cure" to confirm complete infection clearance. This approach is more commonly used in specific populations, such as people experiencing persistent symptoms or pregnant women.

If symptoms persist after taking the prescribed antibiotics, contact your provider immediately. Often, reinfection rather than treatment failure is responsible. Regardless, follow-up medical evaluation is essential. Your doctor will determine the next course of action, which typically involves additional antibiotic therapy.

Side effects of amoxicillin

While amoxicillin does not treat gonorrhea, it does carry known side effects when used for other conditions. Common effects include:

• Upset stomach • Nausea • Vomiting • Diarrhea • Temporary tooth discoloration (browning, graying, or yellowing)

For gonorrhea, patients receive an intramuscular ceftriaxone injection instead. Potential side effects from this injection include:

Which antibiotic is best for gonorrhea?

Ceftriaxone stands as the preferred and most effective antibiotic for gonorrhea. "The best and preferred antibiotic for gonorrhea is ceftriaxone due to its high effectiveness and the widespread antibiotic resistance that has made drugs like amoxicillin obsolete in treating this infection," Dr. January explains.

This recommendation aligns with current CDC guidelines. As Dr. Mandal notes, "It is crucial to adhere to these guidelines to ensure successful treatment and reduce the spread of resistant strains."

Alternative regimens exist for specific situations. According to Dr. Mandal, these options include:

• Ceftizoxime administered intramuscularly • Cefoxitin administered intramuscularly • Cefotaxime administered intramuscularly • Cefixime taken orally

For patients with documented cephalosporin allergies, a combination of oral azithromycin and intramuscular gentamicin may be considered. Cephalosporin allergies are rare, however. "Alternate regimens involving other antibiotics like gentamicin or spectinomycin are preferred in cases of severe allergies to cephalosporins," Dr. January explains.

Amoxicillin is a penicillin-based antibiotic approved by the U.S. Food and Drug Administration (FDA) to treat numerous bacterial infections. These range from ear and sinus infections to lower respiratory tract infections and urinary tract infections (UTIs). It is also used in combination therapies for Helicobacter pylori infections and related ulcer disease.

Given its status as one of the most frequently prescribed antibiotics nationwide, amoxicillin may be among the first medications people consider when diagnosed with a sexually transmitted disease (STD). Although amoxicillin can address some STDs—including chlamydia and gonorrhea—it is typically not the preferred initial treatment. This article explores what current evidence and clinical guidelines say about using amoxicillin for STD management.

Can amoxicillin treat STDs?

Amoxicillin carries FDA approval for treating infections of the nose, throat, ears, lungs, skin, and urinary tract. In some cases, physicians prescribe it off-label for other bacterial infections, including certain STDs.

Antibiotics work by killing bacteria or halting their growth to resolve infection. Amoxicillin belongs to the penicillin family of antibiotics, which function by disrupting bacteria's ability to build and maintain their cell walls. Once bacterial cell walls deteriorate, the organisms cannot survive and are more readily eliminated by the body's immune system.

Despite not being the primary recommended therapy for any sexually transmitted infection, amoxicillin may still demonstrate effectiveness in treating chlamydia, gonorrhea, and syphilis.

Chlamydia

Chlamydia trachomatis is the bacterium responsible for chlamydia infections, which rank among the most prevalent STDs in the United States. The CDC documented more than 1.6 million cases in 2022. Transmission occurs through oral, vaginal, or anal sexual contact and affects both men and women. An infected pregnant woman can also transmit the infection to her infant during delivery.

Many individuals with chlamydia experience no symptoms, which means they remain unaware of their infection, according to CDC data. When left untreated, chlamydia may lead to severe complications—most notably permanent reproductive tract damage in women, potentially compromising their fertility. When symptoms do appear, they may take several weeks to develop and can include pelvic pain, abnormal penile or vaginal discharge, painful urination, or rectal pain, discharge, or bleeding.

The good news is that appropriate oral antibiotics can effectively treat and clear chlamydia, though any structural damage the infection caused cannot be reversed. Doxycycline is the recommended first-line agent for adolescents and adults with chlamydia, with azithromycin or levofloxacin as alternatives.

Amoxicillin is not recommended by the CDC for routine chlamydia treatment in the general population. However, it is recognized as one option for pregnant individuals with chlamydia. The CDC's preferred treatment for pregnant patients is azithromycin, though amoxicillin can serve as an acceptable substitute.

Gonorrhea

Neisseria gonorrhoeae causes gonorrhea, a common STD reported in more than 648,000 U.S. cases in 2022. Transmission occurs through any form of sexual contact involving the mouth, throat, eyes, urethra, penis, or anus. Like chlamydia, maternal transmission to neonates during childbirth is possible.

Many gonorrhea infections produce no symptoms. When symptoms are present, they may include dysuria (burning during urination), unusual discharge from the genitals, intermenstrual vaginal bleeding, or pain in the testicles or scrotum. Whether symptomatic or not, untreated gonorrhea carries serious consequences for both sexes. Women may develop pelvic inflammatory disease (PID) and subsequent infertility. Men can develop epididymitis leading to male infertility. In severe cases, the infection may disseminate to the bloodstream, causing disseminated gonococcal infection—a potentially life-threatening condition.

Gonorrhea is curable with antibiotics, though permanent damage from the infection cannot be reversed. The CDC recommends a single intramuscular dose of ceftriaxone as first-line treatment, with gentamicin, azithromycin, and cefixime as alternatives.

Amoxicillin is not among CDC-recommended therapies for gonorrhea. Nevertheless, certain research indicates amoxicillin may be effective against the pathogen.

Syphilis

Treponema pallidum bacterium causes syphilis. In 2021, more than 176,000 new syphilis cases were reported in the U.S., with approximately 36% of primary and secondary cases occurring in gay, bisexual, and other men who have sex with men (MSM). The infection spreads via direct contact with a syphilitic sore, known as a chancre, which may appear on the penis, vagina, anus, rectum, lips, or mouth. Transmission also occurs during vaginal, anal, or oral sex and from an infected pregnant woman to her fetus.

Syphilis progresses through four distinct stages, with clinical presentation varying by stage:

Primary stage: One or more painless, firm sores develop on or around the genitals, anus, rectum, or mouth. These sores may resolve spontaneously regardless of treatment.

Secondary stage: Skin rashes or oral, genital, or rectal sores develop. The rash often affects the trunk, palms, and soles of the feet, appearing rough and red or reddish-brown.

Latent stage: Few or no visible symptoms occur, yet untreated infection persists for years.

Tertiary stage: Though uncommon, tertiary syphilis may damage the skin, bones, brain, and heart.

Untreated syphilis can also spread to the brain, nervous system, eyes, and ears. Fortunately, appropriate antibiotics can cure syphilis, though any permanent damage is irreversible. According to the CDC, penicillin G benzathine (Bicillin L-A) is the preferred agent for all syphilis stages.

While the CDC does not recommend amoxicillin for syphilis treatment, evidence suggests effectiveness in particular scenarios, including maternal syphilis and co-infection with human immunodeficiency virus (HIV).

Efficacy of amoxicillin for STD treatment

For chlamydia, the CDC endorses doxycycline with azithromycin and levofloxacin as alternatives. In pregnant individuals, azithromycin is preferred, though amoxicillin is accepted as an alternative. A 2001 comparative study of azithromycin versus amoxicillin in pregnant patients showed high cure rates for both drugs. Among study participants who completed post-treatment testing, 20% of those receiving amoxicillin remained chlamydia-positive compared to 5% of those on azithromycin.

Ceftriaxone given as a single intramuscular injection is the CDC-recommended treatment for gonorrhea, with gentamicin, azithromycin, and cefixime as additional options. Although amoxicillin is not the preferred therapy, research demonstrates potential benefit. A 1974 investigation found amoxicillin delivered as a 3-gram single dose achieved a 95% cure rate, rising to 100% when administered in multiple doses over four days.

For syphilis, intramuscular penicillin G is the CDC's first-choice treatment across all disease stages in the U.S. However, other countries, including Japan and the United Kingdom, recommend oral amoxicillin. One study showed amoxicillin combined with probenecid produced a 94.4% cure rate in HIV-positive patients with syphilis. Another study reported a 93.8% cure rate with amoxicillin monotherapy in HIV-positive patients and 100% in HIV-negative patients.

Amoxicillin dosage for STD treatment

For pregnant women with chlamydia, the CDC recommends 500 milligrams of amoxicillin taken orally three times daily for seven days.

Since amoxicillin is not a CDC-recommended first-line agent for gonorrhea or syphilis, no standardized dosage exists for these conditions. If your physician prescribes amoxicillin for your STD, the dose will likely be tailored to your specific situation, considering infection severity, age, concurrent medications, and underlying health conditions.

As with all antibiotics, finishing the complete course is critical, even if symptoms improve. Discontinuing early can allow the infection to return or develop into a more difficult-to-treat form.

Risks and side effects of STD treatment with amoxicillin

Amoxicillin is typically well-tolerated, with most patients experiencing few adverse effects. However, all medications carry risks, and amoxicillin is capable of producing mild to serious side effects.

Common side effects may include rash, diarrhea, headache, nausea, vomiting, stomach pain, tongue discoloration, and yeast infection.

Rare but serious side effects can encompass Clostridium difficile-associated diarrhea, liver dysfunction, blood abnormalities, seizures, aseptic meningitis, and severe allergic reactions.

Most amoxicillin-related side effects are temporary and resolve after treatment completion. However, certain adverse effects—such as blood disorders or C. difficile diarrhea—may require extended time to fully resolve.

Amoxicillin resistance and STD treatment

Antibiotic resistance arises when bacteria evolve mechanisms to evade drugs intended to kill them. This allows pathogens to continue multiplying, making infections progressively harder—and sometimes impossible—to treat.

Gonorrhea has emerged as a major public health concern due to escalating antibiotic resistance. The bacterium has developed resistance to nearly all historically used antibiotics, leaving only a narrow arsenal of effective agents. Resistance in chlamydia and syphilis exists but occurs far less frequently.

Several strategies help mitigate antibiotic resistance:

STD prevention: Reducing STD transmission is fundamental and can be achieved through sexual abstinence, vaccination, or mutually monogamous relationships with uninfected partners.

Regular STD testing: Understanding your STD status and that of your sexual partners is one of the most effective ways to prevent disease spread. Many STDs are curable with appropriate treatment.

Completing full antibiotic courses: When prescribed STD treatment, take the entire course exactly as directed, even after symptom resolution.

Open healthcare communication: If symptoms persist despite treatment, inform your provider immediately, as your infection may have developed antibiotic resistance requiring alternative therapy.

Although metronidazole does not directly eradicate chlamydia or gonorrhea, it addresses certain bacterial components of PID when it arises as a complication of these infections. For PID, metronidazole is typically dosed at 500 mg twice daily in conjunction with other medications selected by the healthcare provider based on the specific organisms involved.

Bacterial vaginosis (BV)

Bacterial vaginosis is a frequent vaginal infection that is not classified as an STI, though its relationship to sexual activity remains incompletely understood. People who forgo condom use or have multiple sexual partners face elevated risk. BV can increase susceptibility to other sexually transmitted infections.

For BV treatment, metronidazole can be taken orally, usually 500 mg twice daily for seven days. Alternatively, a vaginal gel formulation is available—the patient applies one applicatorful vaginally once daily (typically at bedtime) for five consecutive days.

Gonorrhea symptoms

According to the CDC, gonorrhea presentations vary considerably. Some individuals experience no symptoms, while others develop mild to moderate signs. Women may report dysuria (burning during urination), vaginal discharge, or intermenstrual bleeding. Men commonly experience dysuria and penile discharge; testicular pain or swelling is less frequent. Those with rectal gonorrhea may experience anal discharge, pruritus, soreness, rectal bleeding, or painful defecation.

Both the patient and all sexual partners must receive treatment to prevent reinfection. Left untreated, gonorrhea can progress and cause serious sequelae, including PID in women—a major risk factor for infertility—as well as infertility and other complications in men.

Recommended treatments for gonorrhea

The CDC recommends a single intramuscular injection of ceftriaxone as first-line treatment for gonorrhea. When ceftriaxone is unavailable, a single dose of the oral antibiotic cefixime is the alternative. Expedited Partner Therapy (EPT) is also endorsed by public health guidelines, enabling sexual partners of someone with an STI diagnosis to receive medication without undergoing separate clinical evaluation. For gonorrhea, EPT typically involves cefixime monotherapy. If chlamydia co-infection cannot be excluded, providers generally prescribe a seven-day course of doxycycline to both the patient and their partner(s).

How quickly does gonorrhea clear up?

Following antibiotic therapy, gonorrhea typically resolves within 7 to 14 days, though some individuals may experience symptoms beyond this window.
