Is Augmentin (amoxicillin-clavulanate) safe to take during pregnancy?
Yes, for most people — amoxicillin, the primary component in Augmentin, has decades of safety data in pregnancy, and the FDA's older classification system placed it in the lowest-concern category for this reason. "It is safe as long as the patient is not allergic to the medication," says Adi Davidov, MD, associate chair of obstetrics and gynecology at Northwell's Staten Island University Hospital. "Our current evidence shows that this medication is unlikely to cause harm to a developing baby," he adds, pointing to a large body of observational data built up over decades of use. A Danish population-based study found no elevated risk of preterm birth, structural birth defects, or miscarriage among pregnant women prescribed amoxicillin, and a separate study of roughly 7,000 women found no link between amoxicillin use and congenital disorders.
Providers weigh this safety record against a simple fact: an untreated bacterial infection during pregnancy is usually riskier than treating it. "We feel comfortable prescribing it because the alternative — untreated infections — is far riskier to pregnancy than the use of a safe antibiotic like amoxicillin," says Jennifer Lincoln, MD, OB-GYN and author of The Birth Book. Both strep infections and syphilis, for example, can be treated with penicillin-based antibiotics like amoxicillin or Augmentin, and leaving either untreated in pregnancy carries real risk to mother and baby.
The main risk factor isn't the drug itself — it's allergy. "The risks for amoxicillin use would be if the patient has an allergy to either amoxicillin or penicillins, as it is in the penicillin family of antibiotics," explains Mitchell Kramer, MD, OB-GYN and director of obstetrical and gynecologic strategy and clinical integration at Huntington Hospital. Between 8% and 13% of pregnant patients report a penicillin allergy, though roughly 95% of reported allergies turn out not to be genuine once properly evaluated — true penicillin allergy is serious and worth confirming with your provider rather than assuming. Short of an allergy, amoxicillin's most common pregnancy side effects are mild — diarrhea, nausea, and, as Dr. Kramer notes, "a patient can develop a vaginal yeast infection" — and none of these rule out its use. When an alternative is genuinely needed, Dr. Kramer points to cephalosporins like cephalexin or cefuroxime, and erythromycin, as pregnancy-compatible options.
This research base is specific to amoxicillin, the majority component of Augmentin — clavulanate, the second ingredient, hasn't been shown to add meaningful pregnancy risk on top of that. As with any medication in pregnancy, this is a decision to make with your own provider, who can weigh your specific infection and history.
