Are antibiotics available over the counter?

Short answer: not the ones that matter most. Here's the narrow exception, and why the rule exists.

  • Oral and injectable antibiotics cannot be purchased without a prescription in the U.S. because they require proper medical diagnosis, carry risks of misuse, and contribute to antibiotic resistance when used incorrectly.
  • A limited selection of topical antibiotic ointments, creams, and drops—designed for skin, eye, and ear infections—are sold over the counter, though they are not appropriate for serious systemic infections.
  • Some plant-based compounds like honey, garlic, and ginger have shown antimicrobial properties in research, but they cannot replace prescription antibiotics for treating bacterial infections that require medical intervention.

Antibiotics are drugs prescribed by healthcare professionals to eliminate or slow the growth of bacteria, helping your immune system overcome infections such as strep throat, urinary tract infections, skin infections, and sexually transmitted infections. These medications cannot treat viral or fungal infections. While prescription-strength antibiotics taken by mouth or injected must be ordered by a doctor, some topical formulations are accessible without a prescription.

Why are antibiotics prescription-only?

In the United States, systemic antibiotics—medications taken orally or administered by injection—are available only with a prescription. Healthcare providers restrict access for several important reasons.

Accurate diagnosis is foundational. As Walter Gaman, MD, DABFM, FAAFP, a family medicine physician and founding partner of Executive Medicine of Texas in Southlake, Texas, explains: "[Antibiotics] require proper diagnosis to ensure the infection is actually bacterial rather than viral, and appropriate selection based on the specific pathogen and patient factors like allergies and kidney function."

Without medical oversight, antibiotics would be used inappropriately—prescribed for viral infections where they provide no benefit. Chrissie Ott, MD, a dual board-certified integrative pediatrician and internist and founder and CEO of Joy Point Solutions in Portland, Oregon, notes: "If non-clinicians were able to get antibiotics, we would have many incidences of viral respiratory infections treated with antibiotics, and we would see an increase in antibiotic-resistant infections. There would also be legitimate concern for symptoms being treated without adequate evaluation, delaying needed diagnosis and care."

This misuse feeds a serious public health problem: antibiotic resistance. The FDA regulates antibiotic use in humans and animals to combat this growing threat. The agency emphasizes that antibiotics are ineffective against viral infections—including the common cold, flu, and most sore throats—and using them in these situations only accelerates resistance.

Physicians balance treatment benefits against individual patient risks. Dr. Gaman states: "As physicians, we must weigh the benefits against risks for each patient, considering their medical history, current medications, and the likelihood that an antibiotic will actually help their condition."

In most states, pharmacists cannot independently prescribe antibiotics, though some states have expanded pharmacist prescribing authority for specific conditions like urinary tract infections or strep throat through collaborative practice agreements with physicians,” Dr. Gaman explains. “However, pharmacists play a crucial role in antibiotic stewardship by counseling patients on proper use, identifying potential drug interactions, and consulting with physicians when they have concerns about prescribed regimens.

Though systemic antibiotics remain prescription-only, topical treatments and natural alternatives exist that may provide some support for certain conditions.

OTC topical antibiotics: options and alternatives

Dr. Gaman emphasizes that no over-the-counter medication can truly replace injectable or oral antibiotics for treating bacterial infections. However, several options can support immune function and ease symptoms.

Below are over-the-counter topical antibiotics available for certain conditions:

Antibiotic ointments

Over-the-counter topical antibiotic products—available as ointments, creams, and drops—can address specific infections when applied directly to affected areas.

Topical antibiotics fall into three main categories based on their use:

• Dermal products treat skin infections • Ophthalmic products treat eye infections • Otic products treat ear infections

Common over-the-counter options include bacitracin, Neosporin (a combination of bacitracin, neomycin, and polymyxin B), and Polysporin (bacitracin and polymyxin B).

Research demonstrates that for some skin infections, direct application to the affected area can match or exceed the effectiveness of systemic treatment, since topical administration delivers higher drug concentrations where needed. In some cases, healthcare providers recommend non-antibiotic alternatives like petroleum jelly after minor skin procedures, as these alternatives often carry lower risk of side effects such as allergic reactions.

If an infection worsens, you develop side effects, or symptoms return after treatment ends, contact your healthcare provider for further evaluation and potentially stronger treatment options.

Natural remedies

Several plant-derived compounds possess properties resembling those of antibiotics. Dr. Gaman notes: "Natural antimicrobial compounds like honey, garlic, and echinacea may provide some immune support, though their effectiveness is limited and they should never replace proper antibiotic treatment for serious bacterial infections."

Evidence for natural treatments remains sparse, but research has identified potential antimicrobial or immune-supporting properties in several substances:

Ginger: Laboratory studies suggest ginger may inhibit various bacterial strains, though most research has been conducted in vitro rather than in human subjects.

Honey: Studies show honey contains antibacterial and antimicrobial compounds. It is sometimes applied topically to support wound healing.

Garlic: Research indicates garlic may exhibit antibiotic activity against diverse bacteria, including multidrug-resistant strains.

Echinacea: Also called coneflower, echinacea has been investigated for antibacterial potential as a mouthwash and for potentially shortening mild respiratory infections.

Clove: Laboratory testing demonstrated that clove extract showed activity against some bacteria implicated in urinary tract infections.

Oregano: The compound carvacrol, found in oregano, may possess antimicrobial properties. Research has examined its use against Streptococcus mutans, a bacterium associated with tooth decay.

Dr. Ott agrees with Dr. Gaman that natural substances cannot substitute for prescription antibiotics: "When people ask me about over-the-counter alternatives to antibiotics, I'm honest: if it's a real bacterial infection that needs treatment, there's no substitute. Some folks turn to immune-boosting supplements—things like various mushroom species, goldenseal, and others, and while those aren't antibiotics, they may help the body resist infection. I've also seen supplements like D-Mannose used to help prevent bacteria from attaching to the urinary tract, which can lower the chances of getting a UTI."

Prescription antibiotics

All systemic antibiotics—medications taken by mouth or injection—require a prescription from a licensed healthcare provider because they carry substantial risk of adverse effects and potential for misuse.

Common antibiotics prescribed for typical infections include:

Amoxicillin: A penicillin-class antibiotic for bacterial infections of the ear, nose, throat, and urinary system • Azithromycin: Often used for bacterial respiratory, middle ear, and skin infections • Ciprofloxacin: Treats bacterial infections of the skin, bone, joints, lungs, sinuses, and bladder • Doxycycline: A tetracycline antibiotic for bacterial infections, acne, and parasitic infections • Cephalexin: A cephalosporin antibiotic for skin and soft tissue infections

Healthcare providers select antibiotics based on the specific infection and patient factors. Dr. Gaman explains: "When patients have antibiotic allergies or resistance, I may prescribe alternative classes of antibiotics that work through different mechanisms—for example, switching from penicillin to macrolides, cephalosporins, or fluoroquinolones depending on the infection and patient factors."

Dangers of using antibiotics without a prescription

Taking leftover antibiotics, purchasing them online without a prescription, or self-diagnosing based on symptoms carries significant health risks.

According to Dr. Gaman, the hazards include:

• Development of antibiotic resistance • Delayed diagnosis and appropriate treatment • Incorrect dosing that may prove ineffective or toxic • Severe allergic reactions • Interactions with other medications • Disruption of beneficial gut bacteria, potentially leading to secondary infections

Always consult a healthcare provider before starting any antibiotic to minimize the risk of serious or life-threatening complications.
