What is Besifloxacin used for?
Besifloxacin is a fluoroquinolone antibiotic formulated exclusively as an eye drop — unlike most other fluoroquinolones used in ophthalmology (such as moxifloxacin or ciprofloxacin), it was developed specifically for topical ocular use and isn't sold as an oral or IV product for systemic infections. It's FDA-approved for bacterial conjunctivitis caused by a range of susceptible organisms, including several Staphylococcus, Streptococcus, and Haemophilus species.
The eye-drop suspension uses a viscosity-increasing vehicle that keeps the drug in contact with the eye's surface longer than a typical water-based drop, which supports its three-times-daily dosing schedule rather than the more frequent instillation some other topical antibiotics require. Because it isn't used systemically, besifloxacin doesn't carry the class-wide boxed warning that oral and IV fluoroquinolones do (tendon rupture, peripheral neuropathy, CNS effects) — systemic absorption from the eye is minimal.
As with other antibiotic eye drops, besifloxacin only helps if the conjunctivitis is bacterial — it won't treat the more common viral or allergic causes of red, irritated eyes, so getting the right diagnosis before starting a prescription antibiotic drop matters.
