The underlying mechanism involves oxidative stress and mitochondrial injury within muscle cells, leading to impaired neuromuscular transmission, reduced muscle function, and possible muscle tissue loss manifesting as weakness or discomfort.
Because fluoroquinolones are routinely contraindicated in myasthenia gravis, documented cases remain limited. A retrospective analysis identified levofloxacin involvement in nine of 37 reported cases of worsening disease. Importantly, muscle symptoms typically improve rapidly once the antibiotic is stopped. Levofloxacin interactions

Multivalent cation-containing products
Antacids, magnesium supplements, aluminum preparations (including sucralfate), and didanosine all contain multivalent cations that reduce levofloxacin absorption. Separate levofloxacin administration by at least two hours from these products.
Warfarin
Warfarin is an anticoagulant used to prevent thromboembolism. Concurrent use with levofloxacin can intensify anticoagulation, resulting in serious bleeding. Unless prothrombin time can be monitored every 48 hours, levofloxacin is generally avoided in warfarin-treated patients.
Antidiabetic agents
Levofloxacin may raise blood levels of certain diabetes medications. Patients taking oral antidiabetic drugs should inform their healthcare provider before starting levofloxacin and expect close glucose monitoring throughout treatment to prevent dangerous hypoglycemia.
NSAIDs (non-steroidal anti-inflammatory drugs)

Other drug interactions
